Hospice at Home

Hospice at HomeCotswold Care Hospice at Home Service is a team of nurses and health care assistants with skills to provide specialist palliative nursing care to patients in the community. The team offers support for carers by providing episodes of care in situations proving complicated for families and carers.

The Hospice At Home Service allows patients to have options about where they receive their care, even at a time when extra support may be required. For some people remaining in familiar surroundings with family and friends nearby may feel comfortable and reassuring. Being able to exercise choice may be especially important.

Hospice at Home can also make it possible for patients to spend the end stage of their illness in their own homes or another community setting, if they wish. It can help to prevent an unwanted hospital admission.

The Service is available 24 hours a day, 7 days a week, to support the patients and families involved. Working closely with the local Primary Care Team, Clinical Nurse Specialists and Social Care Services, we aim to provide integrated and collaborative care. The team offers nursing support to the patient and family to enable carers to have a break during the day or night.
