Hospice at Home
The
Hospice At Home Service makes it possible for patients to spend
the end stage of their illness in their own homes or another community
setting, if they wish. This can help to prevent an unwanted hospital
admission.
Cotswold Care Hospice at Home Service is a team of
nurses with special skills who provide specialist palliative nursing
care to patients in the community. The nurses can also offer some
much-needed support for carers by providing some shorter episodes
of care in situations, which are proving complicated for families
and carers. The team has the benefit of some trained volunteers
who befriend and offer practical support to the patient and family
to enable carers to have a short break away during the day.
A nurse co-ordinator from the Service is always on call –
24 hours a day, 7 days a week, to support the Primary Care Team,
the Hospice At Home staff, and the families involved. Working closely
with local District Nursing and GP teams, Macmillan Nurses and Social
Services, we provide integrated and collaborative care.
