Guidance Notes for Job Applicants
Please read these notes carefully before completing
the application form.
- Please complete the form in type or black ink.
- Please complete fully the section about previous employment
and include a brief explanation of any gaps in the history.
- Please note in respect of the Sections dealing with Education/Training/Qualifications
that the Hospice will verify qualifications as appropriate by
examining original copies of certificates, diplomas etc.
If you are invited for interview you will be told which of these
will be needed for verification.
- In the section for Skills and Interests please enter those which
are relevant to the post you have applied for.
- In this section please ensure that you relate your response
to the Job Description for the position and the “Person”
Specification, if provided.
- The Disability Discrimination Act 1995 states that “a
person has a disability for the purpose of this Act if they have
a physical or mental impairment which has a substantial and long
term adverse effect on their ability to carry out normal day-to-day
activities”. Cotswold Care welcomes applications from disabled
people and wishes to ensure that they are not disadvantaged at
any stage of the recruitment and selection process and also to
assist them to be able to undertake employment in the Hospice.
Would you please inform us of any special requirements you may
have.
- Please give the names and addresses of two people from whom
references may be obtained. These must be your immediate manager/supervisor
at your present or most recent employer and one other person who
would know you in connection with your work. Please state their
position within the organisation or their relationship to you.
If you are unable to give the names of recent employers as referees,
please state your reasons (e.g. I have not worked for many years).
In this instance please give the names of two other referees and
state in what capacity you are known to them or relationship with
you.
It is normal practice that references are taken up only in respect
of those candidates who are shortlisted for interview and references
will be sought at this stage. It is appreciated that taking up
references at shortlisting stage may cause embarrassment, therefore,
if you do not wish a referee to be contacted at this stage please
indicate as shown.
- Canvassing - Any applicant who canvasses a Director or member
of staff of the Hospice will be disqualified.
- Because of the nature of the work of the Hospice many of its
posts are exempt from the provisions of Section 4 (2) of the Rehabilitation
of Offenders Act 1974 (Exemption) Order 1975. Applicants for such
exempt posts are not entitled to withhold information about convictions
which for other purposes are “spent” under the provisions
of the Act. In the event of employment, failure to disclose such
convictions could result in dismissal. Any information given will
be completely confidential and will be considered only in relation
to an application or a position to which the Order applies.
For certain posts the Hospice will require a disclosure from the
Criminal Records Bureau. This would be sought as part of an offer
of employment. The job recruitment details will identify if the
post you are applying for is “exempt” from spent convictions
and/or subject to disclosure. If shortlisted for interview you
will be invited to provide details separately under confidential
cover to the Chief Executive.
- A complaint - Any complaints that the application for employment
has been unfairly considered may be made in writing to the Chief
Executive within one month of you receiving the result of your
application.
- Return of completed forms - Please return the application to:

Please enclose an S.A.E if you would like an acknowledgement of
receipt of your application.
- Please complete the Equal Opportunities monitoring part of the
application. This will not be available to the person shortlisting
and is essential to helping the Hospice maintain the operation
and development of its equal opportunities objectives in practice.
